Arc of the Three Rivers provides Conflict-Free Case Management services to help individuals with developmental and intellectual disabilities access and coordinate the supports they need to live full, meaningful, and self-directed lives. This service is built on a person-centered, goal-oriented approach that recognizes each individual’s strengths, needs, preferences, and vision for the future.
Conflict-Free Case Management involves working closely with the individual, family members, caregivers, and other members of the support team to coordinate both natural and paid supports. These may include family and community resources, healthcare services, educational supports, residential services, employment supports, and other programs that contribute to the individual’s well-being and success.
Our case management team helps ensure services are organized, accessible, and responsive to changing needs over time. We assist with planning, problem-solving, advocacy, service coordination, monitoring progress, and helping individuals make informed choices about the supports that best fit their lives.
Because this service is conflict-free, the focus remains on what is best for the person receiving services. This helps promote accountability, continuity, and trust throughout the planning process while supporting each individual’s right to choice, independence, and inclusion in the community.
